Centura Health at Home offers adult palliative care for those facing chronic and serious illnesses. Our team of professionals offers coordinated and comforting care to address a patient's physical, psychological, social and spiritual needs. The goal is to prevent and relieve suffering and provide the best quality of life for patients. Palliative care services are available to patients wherever the patient calls home, which could be in their own residence, a nursing home or other residence.

The Centura Health at Home Foundation helps support palliative care patients who are unable to pay for services or underinsured. Patients may also need special equipment like a wheel chair ramp installed in their home or other necessities to make their home safe. You can ensure a patient with a serious illness receives the care and equipment they need by making a donation today.
